Health Care Proxy - A Health Care Proxy allows a
representative of your choosing to make health care decisions for you if you
become incapable of making those decisions yourself. By appointing a health care agent, you can make sure that health care providers follow your wishes.
Your agent can also decide how your wishes apply as your medical condition changes. The
Health Care Proxy is often combined with the Living Will to create a single
"Advance Directive" document.
Living Will - A Living Will is a legal document you complete to outline the circumstances in
which you would or would not accept certain kinds of life-prolonging medical treatments. You can also use a Living Will to list other treatments as secondary measures.
